Clinical Outcomes Trending Field Descriptions

Summary

This report compares the current quarter to the previous quarter and it calculates patient improvement on average from each outcome tool. It also breaks down changes for each individual therapist. Only OMTs that have a complete data set will be counted here.

Note: A complete data set is comprised of a completed outcomes survey on the Initial Evaluation and Discharge Summary. However, if the case has a Progress Note with a completed outcomes survey and the patient was Quick Discharged, the system will convert the Progress Note Survey to a Discharge survey. This ensures a complete data set for patients who self-discharge, or are otherwise unable to complete their course of care.

  • Initial: This is the average score achieved in all of the Initial Evaluations for the reporting period.
  • Completion of Care: This is the average score achieved for the OMT at Discharge Summaries for the reporting period.
  • Difference: The difference between Initial and Completion of Care.
  • Percentage Change: This is the percent difference between Initial and Discharge.
  • Green Bars: The green bar highlights the period when best results were achieved. If there are two green bars on the same line it means that they had the same results.
