Measure 182: Functional Outcome Assessment (SOAP 1.0)

Standardized outcome assessments, questionnaires, or tools are a vital part of evidence-based practice. Using standardized tests and measures early in an episode of care establishes the patient’s baseline status and provides the ability to quantify the change in the patient's functioning.

Note
  • Quality measures must be documented in both WebPT and Keet. The data in Keet is collated and sent to CMS during the MIPS submission period, and the data in the WebPT EMR supports what is sent to CMS.
  • Previously, Members reporting for MIPS with WebPT selected which measures they wanted to collect data for during the setup process. This step has been eliminated. You can now select which MIPS measures to collect data for on a visit by visit basis.

Who can report this measure?

PTs, OTs, and SLPs can report on this measure.

Patient Qualifications

  • Patient age (18 or older)
  • Visit Type: Initial Evaluation, Re-Evaluation, and Discharge Summary

When do I report on this measure?

Measure 182 is reported at each eligible visit type or at least every 30 days, for patients seen during the performance period.  

Measure Requirements

  • Documented functional outcome assessment using a standardized functional outcome assessment tool (a tool that has been normed and validated)
  • Documentation of a Care Plan based on identified functional outcome deficiencies on the date of the identified deficiencies

How to Report Measure 182

Measure 182 requires the completion of a functional outcome assessment by a patient. WebPT Members can administer functional outcome assessments in two ways:

  • Send an Outcome Measurement Tool (OMT) through the WebPT EMR
  • Send a Patient Reported Outcome (PRO) through Keet

Either of these will satisfy the requirements needed to successfully report Measure 182, but there are some differences in how they’re documented. We’ve outlined the steps for both processes below.

Using WebPT to Administer an OMT

On the Objective tab, select any OMT test that is current and functional.

Note: WebPT Outcomes supports administering the following OMT’s for Measure 182: Bates-Jensen Wound Assessment, Foot and Ankle Ability Measure (FAAM)- Sports Subscale, HOOS, KOOS, Lymphedema Life Impact Scale, Modified Falls Efficacy Scale, and Pelvic Floor Distress Inventory Short Form 20.   1. Search OMT in the Add Tests box to display a complete list, or search for the desired OMT. Alternatively, pair the OMT that has been completed by the patient.
  2. In this example, we’ll select the Hand Profile, as our patient presented with a hand-related diagnosis. Use the Yes radio button in Upper Extremity to reveal the OMT, then use the Yes radio button next to Hand Profile to open the test.
  3. Next, administer the OMT. You can use the Print Test functionality to provide the survey to the patient and enter the score into the text box when complete.
  4. Given the patient’s result, you should tailor your documentation in the Assessment and Plan tabs to address the patient’s functional definitions including Treatment to be provided and Goals. This information should form the required Care Plan.
  5. When finished documenting, proceed with the steps in the Keet: Reporting Selections section.

Using Keet to Administer a PRO

The PRO’s in Keet associated with IROMS Measures can also be used to satisfy Measure 182’s requirements. However, you cannot use one PRO to satisfy the requirements for both measures at the same time. If you’d like to use a PRO for Measure 182, review the steps below:

Note: Keet supports administering the following PRO’s for Measure 182: Lower Extremity Functional Scale, Knee Outcome Survey, Neck Disability Index, Modified Oswestry Low Back Pain Disability Questionnaire, Quick Dash, and Dizziness Handicap Index. 

  1. In Keet, edit the Primary Classification of the episode of care to Other. This will allow you to use a PRO to satisfy Measure 182, while also taking the patient out of the denominator for IROMS Measures, meaning that it will not count against your score if you do not collect IROMS measure data for this patient. 

  2. Add the desired PRO to the patient’s Care Plan.

  3. Deliver the questionnaire to the patient by presenting them with a QR Code or sending an email, or complete it with the patient during the visit.

  4. Once the questionnaire is complete, proceed with the steps in the Keet: Reporting Selections section.

Keet: Reporting Selections

In Keet, you’ll need to add this activity to the patient’s Care Plan.

Note: The data in Keet is collated and sent to CMS. The data in the WebPT EMR supports what is submitted to CMS in the event of an audit.
  1. Use the MIPS Badge on the patient chart or the MIPS tab in the note to open Keet.
  2. Log in to Keet.
    Note: After logging in to Keet, keep the tab open to remain logged in to Keet to streamline this step while documenting on future appointments. Keet may log users out after three hours of inactivity. 
  3. In Keet, navigate to the patient’s Episode of Care that corresponds with the case in the WebPT EMR.
  4. Click Add Activity.
  5. Click Measure 182: Functional Outcome Assessment to add to the patient’s Care Plan.
  6. In the Care Plan, click the ellipses (...) icon in the Action column.
  7. Click Complete.
  8. Select the option that best describes your supporting documentation.
  9. Click Submit.
