Measure 134: Preventive Care and Screening: Screening for Depression and Follow-Up Plan (SOAP 1.0)

Depression is a serious medical illness associated with higher rates of chronic disease, increased health care utilization, and impaired functioning (Pratt, Brody 2014). U.S. survey data (2014) indicates that 2.8 million (11.4 percent) adolescents aged 12 to 17 had a major depressive episode (MDE) in the past year and that 15.7 million (6.6 percent) adults aged 18 or older had at least one MDE in the past year.

Note
  • Quality measures must be documented in both WebPT and Keet. The data in Keet is collated and sent to CMS during the MIPS submission period, and the data in the WebPT EMR supports what is sent to CMS.
  • Previously, Members reporting for MIPS with WebPT selected which measures they wanted to collect data for during the setup process. This step has been eliminated. You can now select which MIPS measures to collect data for on a visit by visit basis.

Who can report this measure?

PTs and OTs can report this measure.

Patient Qualifications

  • Patient age (12 or older)
  • Patient does not have an active diagnosis of depression
  • Patient has not been diagnosed with bipolar disorder
  • Visit type: Initial Evaluation

When do I report on this measure?

During each eligible visit within the performance period.

Measure Requirements

1) Screen the patient using one of the following Outcome Measurement Tools based on age:

  • 12-17 – Mood and Feeling Questionnaire
  • 18+ – Geriatric Depression Scale
  • Alternatively, Other Depression Scale may be used if a different standardized instrument will be used for depression screening

2) If the depression screening is positive, then you must have a follow-up plan that includes details regarding one or more of the following options:

  • Additional evaluation or assessment for depression
  • Suicide Risk Assessment
  • Referral to a practitioner who is qualified to diagnose and treat depression
  • Pharmacological interventions
  • Other interventions or follow-up for the diagnosis or treatment of depression

Measure Reporting in WebPT

  1. On the Objective Tab, select the correct OMT for the patient based on their age from Measure 134 in the MIPS Measure Tests section. For this example, we’ll select the Geriatric Depression Scale.
  2. In the Outcome Measurement Tools section, select the Yes radio button next to General Function, then Yes for the Geriatric Depression Scale.
  3. Use the Show Test link to enter the patient’s results or simply key them into the entry box.
  4. If the patient’s answers indicate a positive result, you must complete the Follow-Up Plan field following the guidelines listed above.
  5.  When finished documenting, validate the selection in Keet. 

Mood and Feeling Instructions

If you are reporting for a patient aged 12-17, you would select the Mood and Feeling Questionnaire in  Step 1 above, and then follow the instructions below.

  1. In the Outcome Measurement Tools section, select the Yes radio button next to Pediatrics.
  2. Then, choose the Yes radio button for Mood and Feeling Questionnaire.
  3. Use the Show Test link to enter the patient’s results or simply key them into the entry box. 
  4. If the patient’s answers indicate a positive result, you must complete the Follow-up Plan field following the guidelines listed above.

Other Depression Scale Instructions

If you need additional flexibility when applying which depression screening instrument will be used, you would select the Other Depression Scale in Step 1 above, and then follow the instructions below.

  1. In the Outcome Measurement Tools section, select the Yes radio button next to Other Depression Scale.
  2. Then, enter the Depression Screening Instrument you have selected. For this example, let’s enter Beck Depression Inventory (BDI).
  3. Key the patient’s score in the entry box.
  4. Then, select the appropriate radio box to indicate a negative or positive result.
  5. If the patient’s answers indicate a positive result, you must complete the Follow-Up Plan field following the guidelines listed above.

Keet: Reporting Selections

In Keet, you’ll need to add this activity to the patient’s Care Plan.

Note: The data in Keet is collated and sent to CMS. The data in the WebPT EMR supports what is submitted to CMS in the event of an audit.
  1. Use the MIPS Badge on the patient chart or the MIPS tab in the note to open Keet.
  2. Log in to Keet.
    Note: After logging in to Keet, keep the tab open to remain logged in to Keet to streamline this step while documenting on future appointments. Keet may log users out after three hours of inactivity. 
  3. In Keet, navigate to the patient’s Episode of Care that corresponds with the case in the WebPT EMR.
  4. Click Add Activity.
  5. Click Measure 134: Preventive Care and Screening: Screening for Depression and Follow-Up Plan to add to the patient’s Care Plan.
  6. In the Care Plan, click the ellipses (...) icon in the Action column.
  7. Click Complete.
  8. Select the option that best describes your supporting documentation.
  9. Click Submit.
