Measure 128: Body Mass Index (BMI) Screening and Follow-Up (SOAP 1.0)

BMI numbers outside of normal parameters are determined to population health, regardless of whether the patient is above or below the expected range. Screening for BMI and follow-up, therefore, contributes to quality goals of population health and cost reduction.

Note
  • Quality measures must be documented in both WebPT and Keet. The data in Keet is collated and sent to CMS during the MIPS submission period, and the data in the WebPT EMR supports what is sent to CMS.
  • Previously, Members reporting for MIPS with WebPT selected which measures they wanted to collect data for during the setup process. This step has been eliminated. You can now select which MIPS measures to collect data for on a visit by visit basis.

Who can report this measure?

PTs and OTs can report this measure.

Patient Qualifications

  • Patient age (18 or older)
  • BMI is less than or equal to 18.5 and greater than 24.9 kg/m2
  • Visit type: Initial Evaluation

Do I have to report on this measure for every visit?

During each eligible visit within the performance period. If more than one BMI is submitted during the measurement period, then the most recent BMI will be used to determine if the performance has been met. 

Measure Requirements

  • Patient height and weight (measured by staff; self-reporting values not allowed)
  • Patient BMI
  • If the BMI is below or above normal for the patient’s age, then you must have a follow-up plan that includes details regarding one or more of the following options:
  1. Education – counseling you provided regarding weight management and/or resources available to the individual for further education.
  2. Referral to a Registered Dietitian Nutritionist (RDN), exercise physiologist, mental health professional, or surgeon, for lifestyle/behavior therapy
  3. Referral to primary care provider – specific physician referred to.
  4. Dietary supplements
  5. Exercise counseling
  6. Nutrition counseling

Measure Reporting in WebPT

  1. On the Objective Tab, select the Body Mass Index link from Measure 128 in the MIPS Measure Tests section.
  2. In the Inspection section, select the Yes radio button next to Body Mass Index.
  3. Enter the patient’s Weight and Height. Remember, these must be measured in the clinic (not self-reported).
  4. The EMR will calculate the Index number.
  5. If the patient is outside of the expected parameters, complete the Followup Plan field.
  6. When finished documenting, validate the selection in Keet.

Keet: Reporting Selections

In Keet, you’ll need to add this activity to the patient’s Care Plan.

Note: The data in Keet is collated and sent to CMS. The data in the WebPT EMR supports what is submitted to CMS in the event of an audit.
  1. Use the MIPS Badge on the patient chart or the MIPS tab in the note to open Keet.
  2. Log in to Keet.
    Note: After logging in to Keet, keep the tab open to remain logged in to Keet to streamline this step while documenting on future appointments. Keet may log users out after three hours of inactivity. 
  3. In Keet, navigate to the patient’s Episode of Care that corresponds with the case in the WebPT EMR.
  4. Click Add Activity.
  5. Click Measure 128: Body Mass Index (BMI) Screening and Follow-Up to add to the patient’s Care Plan.
  6. In the Care Plan, click the ellipses (...) icon in the Action column.
  7. Click Complete.
  8. Select the option that best describes your supporting documentation.
  9. Click Submit.
