Measure 126: Diabetes Mellitus: Diabetic Foot and Ankle Care, Peripheral Neuropathy: Neurological Evaluation (SOAP 1.0)

Measure 126 is about preventing amputations for patients diagnosed with diabetes. Foot ulceration is the most common single reason for individuals with diabetes to need a lower extremity amputation. A timely neurological evaluation can detect the development of peripheral neuropathy early so that skin damage can be avoided.

Note: 
  • Quality measures must be documented in both WebPT and Keet. The data in Keet is collated and sent to CMS during the MIPS submission period, and the data in the WebPT EMR supports what is sent to CMS.
  • Previously, Members reporting for MIPS with WebPT selected which measures they wanted to collect data for during the setup process. This step has been eliminated. You can now select which MIPS measures to collect data for on a visit by visit basis.

Who can report this measure?

PTs can report this measure.

Patient Qualifications

  • Patient age (18 or older)
  • Visit type: Initial Evaluation or Re-Evaluation
  • Performance of neurological exam of lower extremities within the last 12 months
  • A diagnosis code for diabetes is needed. This may seem like an obvious statement, but most measures that therapists are eligible to report on do not have a diagnosis code requirement. This measure description lists all of the eligible ICD-10 codes that trigger reporting.

When do I report on this measure?

During each eligible visit within the performance period for patients with a qualifying diagnosis (e.g Diabetes Mellitus). Evaluation frequency is decided by the Risk Profile determined during the examination, which is included in the Measure Specification Guide.

Measure Requirements

A mix of clinical actions is needed to meet the measure’s intent. Tests or measures must capture sensory AND motor function in the legs:

  • A 10-g monofilament test of specified areas of the feet is required
  • You must also perform one or more of the following: vibration test using 128-Hz tuning fork, pinprick sensation test, ankle reflexes, or a vibration perception threshold test
  • You also should perform any other examination techniques as indicated by the specific patient’s presentation

Measure Reporting in WebPT

  1. On the Objective tab, add the Semmes Weinstein Lower AND at least one additional test from the MIPS Measure 126 section. All added tests will appear in the Neuro-Vascular section.
  2. In the Neuro-Vascular section, select the Yes radio button next to the Semmes Weinstein Lower AND the additional test. Administer each test to satisfy measure requirements.
  3. When finished documenting, validate the selection in Keet.

Keet: Reporting Selections

In Keet, you’ll need to add this activity to the patient’s Care Plan.

Note: The data in Keet is collated and sent to CMS. The data in the WebPT EMR supports what is submitted to CMS in the event of an audit.
  1. Use the MIPS Badge on the patient chart or the MIPS tab in the note to open Keet.
  2. Log in to Keet.
    Note: After logging in to Keet, keep the tab open to remain logged in to Keet to streamline this step while documenting on future appointments. Keet may log users out after three hours of inactivity. 
  3. In Keet, navigate to the patient’s Episode of Care that corresponds with the case in the WebPT EMR.
  4. Click Add Activity.
  5. Click Measure 126: Diabetes Mellitus: Diabetic Foot and Ankle Care, Peripheral Neuropathy: Neurological Evaluation to add to the patient’s Care Plan.
  6. In the Care Plan, click the ellipses (...) icon in the Action column.
  7. Click Complete.
  8. Select the option that best describes your supporting documentation.
  9. Click Submit.
