MIPS Feedback Reports

To access your payment adjustment information for the 2019 performance period, you must create a HARP account to access the QPP data reporting system. 

To help get you started, we recommend starting with the following:

Register for a HARP Account

Before signing into qpp.cms.gov, create an account in the HARP (HCQIS Access Roles and Profile). HARP is a secure identity management portal provided by CMS.

Note: If you already have a verified EIDM account, you do not need to register for a HARP account. Sign in here with your EIDM User ID and password.
  1. Navigate to qpp.cms.gov and click Sign In in the upper right-hand corner.
  2. If you are a returning user, sign in with the same credentials you’ve always used.
     
    If you are a new user, select the Register tab to create a HARP account.
  3. Select Register with HARP at the bottom of the page to complete registration.
  4. Enter your Profile Information. All fields with an asterisk (*) are required.
    Note: Personal information, like your Home Address and Social Security Number, are necessary in order to verify your identity through Experian’s remote proofing process.
  5. Experian Reference Number: If you have already called Experian, select Enter Reference Number to continue the registration process.
  6. After agreeing to the Terms & Conditions, select Next.
  7. Next, create a unique User ID and Password.
    1. User ID: must be a minimum of 5 alphanumeric characters and symbols
    2. Password: must be a minimum of 12 characters including:
      1. A lowercase letter
      2. An uppercase letter
      3. A number (0=9)
      4. A symbol (., !, @, #, $, %, ^, &, *)
  8. Use the drop-down to select a Challenge Question and provide an answer. Then click Next.
  9. Experian will create personalized remote identity proofing questions based on your Profile Information. Answer the set of questions, then click Next.
  10. After confirmation that your HARP account has been created, click Login to Complete Setup.
  11. Enter your login credentials, then click Login.
  12. Next, set up Two-Factor Authentication. This is a required step for additional security.
  13. After completing your account setup, you can sign in to QPP or sign in to your Harp user profile.

Connect to an Organization

After connecting to an organization, you can access the Security Official or Staff User Role. These roles allow you to view performance feedback or perform MIPS-related actions such as completing an opt-in election or applying for an exception. 

  1. Navigate to qpp.cms.gov and click Sign In in the upper-right hand corner.
  2. Enter your User ID and Password, then click Sign In.
  3. In the left-hand navigation pane, select Manage Access and click Connect to an Organization.
  4. Use the radio buttons to select the Organization Type.
    Note: If you are connecting to more than one organization, you must complete a request for each organization.
  5. Next, select a Role. If you are the first person to connect to your organization, select Security Official. Each organization must have at least one Security Official role before requesting additional roles.
  6. Provide additional information to validate your credentials. The additional information will depend on your selected Organization Type.
  7. After the required information is validated, you have successfully created your Security Official role. You can begin monitoring pending requests for additional Security Official roles and Staff User roles.

Connect as a Clinician

The Clinician role allows you to view your MIPS performance feedback, eligibility details, and APM incentive information relevant to your participation in the QPP.

  1. Navigate to qpp.cms.gov and click Sign In in the upper-right hand corner.
  2. Enter your User ID and Password, then click Sign In.
  3. In the left-hand navigation pane, select Manage Access.
  4. If you have no Connected Organizations, click here for Clinician access.
    If you have one or more Connected Organizations, click Connect to another organization.
  5. Use the radio button to select Individual Clinician.
  6. The name you use to sign in to qpp.cms.gov will be used to verify your identity. If there is an exact match between your HARP account and your name on the QPP Participation Status Lookup Tool, a list of your associated practices will display. Click Verify.
  7. After verification, you can view your roles (Clinician, Staff User, or Security Official) for all connected practices.
