Notes by Clinic

Notes by Clinic is a Data Table style report designed to help you monitor productivity by Clinic, Therapist, and Date of Service and includes SOAP 1.0 and 2.0 data. This data is derived from finalized billable notes and does not include quick discharge note types.

Location

You can locate this report in the Notes section of Reports in Analytics. If you do not see the report listed in this section, ensure that you have the correct Analytics Access permission.

Run and Customize the Report

You can filter which clinics appear in the report using the View Clinics by: drop-down menu and corresponding (…) button.
Use the View Data By drop-down to add additional columns and levels of detail to the table, including the Therapist and Date of Service.
The percent of the total column should add up to 100%.

Tip: When viewing the data by the Therapist, the therapist who receives credit for the note is the documenting therapist. The documenting therapist is the provider who forwarded the note or, if the note was not forwarded, the Documenting Therapist is the provider who finalized the note. This ensures that assistants get credit for the notes that they complete.

Column Field Descriptions

Clinic Name	The clinic location where the patient's note was finalized.	Patient Notes
Therapist (available when View Data By is Therapist)	The documenting therapist, either the provider who forwarded the note or, if the note was not finalized, the provider who finalized the note.	Patient Notes
Days of Service	The total number of dates of service within the selected date range, as determined by finalized notes.	Date of Service field in the finalized SOAP Note
Orthosis Fabrication	The number of Orthosis Fabrication and Orthosis Fabrication Daily Notes finalized within the date range.	Patient Notes
Initial Evaluation	The number of Initial Evaluation notes finalized within the date range.	Patient Notes
Daily Note	The number of Daily Notes finalized within the date range.	Patient Notes
Progress Note	The number of Progress Notes finalized within the date range.	Patient Notes
Re-Evaluation	The number of Re-Examination notes finalized within the date range.	Patient Notes
Discharge Summary	The number of Discharge Summary notes finalized within the date range. This excludes Quick Discharge note types.	Patient Notes
Total Visits	The total number of finalized, billable notes within the date range.	Patient Notes
Avg. / Days of Service	The average number of notes finalized for each date of service.	Calculated Value
