Add Patient: Contact Info

Patient Contact Info is located in the patient chart under Patient Info.

Important: At least one form of contact is required, such as a phone number or email address. Members using the Keet Data Feed should add a valid email address to allow for patient enrollment and accurate workflows in Keet.

You'll find the Contact Info tab nestled between the Address and Identifications tabs.

Phone

When adding phone numbers, you'll be required to add:

  • Contact Type
  • Phone Type: Make sure to select the appropriate phone type, especially for a mobile phone type. This is helpful for various WebPT integrations, such as Reach.
  • Phone Number
Note: You can add as many phone numbers or email addresses as needed. You should check the Set as Primary option if you have multiple contact methods entered. If you only have one contact method entered, it defaults to primary.

Email

When adding phone numbers, you'll be required to add:

  • Contact Type
  • Email

Note: Use the Additional Info field for notes, like the best time to call or contact the patient.

Integrations

Therabill

  • The first email address entered for the patient will integrate, even if it’s not marked as Primary.
  • The first Home phone number entered for the patient will integrate, even if it’s not marked as Primary.
  • Only the address marked as Primary integrates.

WebPT Billing

  • Only the Phone Number marked as Primary integrates.

Keet
