Add Outcome Measurement Tool

Outcome Measurement Tools (OMTs) are used to objectively establish a patient's functional baseline at initial evaluation, measure changes in function throughout treatment, and indicate if predicted outcomes are reached. They allow for continual improvement of the quality of care therapists provide to patients. Medicare and some other insurances require therapists to complete an outcome measurement tool on every patient evaluation before they provide reimbursement.

OMTs are found on the Objective tab of evaluative notes: Initial Examination, Progress Note, Recertification, Re-Examination, and Discharge Summary. You can use specific objective functional outcome measures consistently throughout each episode of care by administering functional outcome measures at the initial evaluation, re-evaluation/progress report, and discharge.

OMTs vary depending on the evaluation profile being used for each patient. You can also use the Add Tests box to search for a test and include it in a note. Type in a search term and click the arrow button.

Add a Custom OMT

When you need to add a test that is not available in the WebPT EMR, follow these instructions to add a Custom OMT:

  1. In the Add Tests box search for Custom Outcome Measurement Tool and use the play button to add.
  2. In the Outcome Measurement Tools section, select the Yes radio button next to Custom Outcome Measurement.
  3. Next, enter the Name of the Outcome, Comments, and the patient’s Score.
  4. Next, click the eDoc link. This opens the patient’s eDocs in a new window (check your pop-up blocker if you do not see it). Upload any supporting documentation for this OMT, such as the patient’s survey results. 

Error Message 

When treating Medicare patients, you must include an outcome measurement score. If you fail to complete at least one outcome, you will see the following error message:
